Confidential Vaccine Declaration Follow-up  

On May 14, the Executive Cabinet announced that the University is requiring every member of the Seton Hall community to be fully vaccinated prior to the start of the fall 2021 semester, subject to exemptions for religious beliefs, pre-existing health conditions, or personal reasons while COVID-19 vaccines are authorized on an emergency-use basis.

To assess the immunization level in our community, we are also requiring each member of our University community to complete a Confidential Vaccine Declaration. 

If you have not yet done so, please take a moment to complete the declaration now. You will be prompted to complete the confidential declaration when you log in to your PirateNet account or you may do so by clicking on the button below.

For those that have already completed the declaration, please note the following:

  • If you responded that you are fully vaccinated, no further action is requited at this time. Please note that you may be asked to provide proof of vaccination should Seton Hall collect additional information in the future.
  • If you responded that you are partially vaccinated, having received one of the two required shots, we will be following up with you to confirm that you received the second vaccination and are now fully vaccinated.
  • If you responded that you had not yet been vaccinated but planned to do so, we will be reaching out to you for an update on how far you have progressed in your efforts to become fully vaccinated.
  • If you responded that you are requesting an exemption due to your religious beliefs, a pre-existing health condition or for personal reasons while the COVID-19 vaccine is authorized an emergency-use basis, the University will be in contact to follow-up regarding your exemption.

Widespread vaccinations provide the clearest and shortest path to resuming activities we enjoy and reaching a high vaccination level in our University population is an important factor in achieving the primarily in-person fall semester we are planning.
