Title IV Certification Acknowledgment
In accordance with Title IV of the Higher Education Act all students are advised that the College's programs leading to licensure, certification, and/or endorsement meet the requirements of New Jersey. Information on out-of-state reciprocity is available through the New Jersey Department of Education and Division of Consumer Affairs websites. Employment and certification requirements vary by state and students interested in out-of-state employment may need to pursue supplemental coursework.
All students enrolled in a program leading to certification are required to complete the Certification Reminder – Student Acknowledgment.

Who needs to sign the acknowledgment?
Every CHDCM student enrolled in a program leading to certification needs to sign the
acknowledgement.
Why do I need to sign the acknowledgment?
In accordance with Title IV of the Higher Education Act all students must be advised
that the College's programs leading to licensure, certification, and/or endorsement
meet the requirements of New Jersey. The acknowledgment ensures that students have
received the information as required by Title IV of the Higher Education Act.
What happens if I do not sign the acknowledgment?
Students who do not sign the acknowledgment by the deadline given will receive a hold
on their student account. The hold will be lifted once the acknowledgment is received.
Will I need to sign the acknowledgment every semester/year?
No, you only need to sign it once.
