General Service Fees

There is no co-pay at time of visit. Insurance will be billed for services.

Outside charges can occur if the following services are provided:

  1. Laboratory (i.e. blood tests, cultures) – a bill will be sent from the outside laboratory to what ever billing address is provided.
  2. Pharmacy – your bursar account will be charged (limited prescriptive medications at reduced cost are available).

Loan items will be charged to your bursar account if they are not returned within two weeks. These items include:

  1. Pair of crutches - $75.00
  2. One crutch - $40.00

All fees and insurance claims are the responsibility of the patient.
