Health Requirements

 
Seton Hall University health requirements apply to all registered matriculated students. Please complete the Health Form prior to enrolling. Incomplete forms will delay your ability to register for class.

Immunization Requirements

Guidelines are established based on New Jersey law (NJAC 8:57-6), Center for Disease Control (CDC) guidelines, Advisory Committee on Immunization Practices (ACIP) guidelines, American College Health Association (ACHA) guidelines, and University policies.

Completion of the Health Form should document the following:

  1. Measles* vaccination: proof of two doses after age one and thirty days apart, or laboratory proof of immunity if born after 1956.
  2. Mumps* vaccinations: proof of two or laboratory proof of immunity if born after 1956.
  3. Rubella* vaccinations: proof of two after age one or laboratory proof of immunity if born after 1956.
  4. Hepatitis B** vaccines (if enrolled in 12 or more credits): proof of three.
  5. Tetanus** vaccine within past ten years.
  6. Meningococcal Meningitis** vaccine (if newly living in campus housing): within five years. 
  7. Tuberculosis risk assessment and follow up as indicated.

  2. Human Papillomavirus** 
  3. Influenza (Flu)**
  4. Hepatitis A**
  5. Pneumococcal (Pneumonia/for high risk groups)
