Health Requirements 

Seton Hall University health requirements apply to all registered matriculating students that attend class on the main campus, Law School Campus and/or congregate at other locations in the name of the University.  Please select the form that applies to you and submit by the date indicated. Incomplete forms will not be accepted and will affect your registration status.

Pre-matriculation immunization requirements:

Pre-matriculation immunization requirements are established according to the New Jersey Vaccine Preventable Disease Program, Higher Education Immunization Subchapter 6 (N.J.A.C. 8:57-6) and Seton Hall University Policies per recommendations of the Centers for Disease Control Advisory Committee on Immunization Practices.

  1. MMR [Measles,Mumps,Rubella]: 2 doses with first dose given on or after first birthday and second dose no sooner than 1 month after first dose.
  2. Tetanus-Diptheria or Tdap: Completion of primary series, documentation of booster within last 10 years.
  3. Polio: Documentation of completed primary series of polio vaccine. if available.
  4. Tuberculosis: Mantoux [PPD] skin test if determined to be high risk. See applicable Health Form.
  5. Meningitis: Documentation of having received Meningococcal vaccine, a housing requirement.
  6. Hepatitis B:  Completion of primary series (3 doses).

Recommended vaccines:

  1. Varicella [chickenpox]
