At this time, every faculty member and employee is required to respond to the Dependent Eligibility Verification Program (see communication dated April 14, 2009) and provide proof of dependent status. If you've already given required documentation to Human Resources for the Tuition Remission benefits or a recent Medical or Dental plan conversion, you will not need to resubmit to auditors.
Any individual for whom you do not provide proof of dependent status will lose medical and dental coverage in accordance with plan provisions. Tuition benefits will be removed for those without proof of dependent status.
Ineligible dependents are not entitled to COBRA continuation coverage. However, if your eligible dependent lost that status within the last 60 days, you must notify Babette Brooks at (973) 275-2755 to request a COBRA Qualified Event Notice.
It is your responsibility to ensure individuals listed as covered dependents met, and continue to meet, the requirements for eligible dependents. Seton Hall University will periodically audit dependent coverage on an ongoing basis.

Based on each dependent's eligibility, identify and locate the documents needed to verify dependent eligibility.
