candidates in field internships and culminating clinical placements is
provided here.
**Please note: The evaluation forms listed here
are provided for review. The required evaluations are provided to you,
in a scannable format, in the information packet you receive at the
convocation held at the start of each semester.
Interns
- Supervisor Handbook
The Handbook provides information for all University Supervisors working with undergraduate candidates in any of these programs: Elementary/Special Education, Speech and Language Pathology, and Secondary Education. The requirements and expectations for the four field experiences are detailed in the handbook.
- Early Warning Form
If a candidate is having difficulty or not responding to structured feedback within the first 35 hours of their field experience, please fill out this form and contact the Office of Field Placement and Supervision. Please provide specific documentation.
Elementary & Special Education/Speech & Language Pathology
Secondary Education
- Secondary Program Preclinical Evaluation Form
Mandated tutoring evaluation form must be completed by the University Supervisor for both semesters of tutoring (Preclinical Field 1 and 2).
- Final Junior Teaching Clinical Evaluation Form
Mandated for to be completed by the Cooperating Teacher and University Supervisor at the completion of the junior 60 hour internships (3 and 4).

both 60 hour internship placements.

Mandated form to be completed by Cooperating Teachers and University

both 60 hour internship placements.

Mandated form to be completed by Cooperating Teachers and University
Supervisors for all Secondary Education Social Studies majors at the end of

- World Language (ACTFL Standards)
Mandated form to be completed by Cooperating Teachers and University
Supervisors for all Secondary Education World Language majors at the end of
both 60 hour internship placements.
Student Teachers
- Supervisor Handbook
The Handbook provides essential information for University Supervisors working with all candidates in their culminating clinical experience. Candidates may be in the Elementary/Special Education, Speech-Pathology Program, Secondary Educations or EPICS. The requirements and expectations for the clinical experience are detailed in the handbook.
- Senior Clinical Evaluation Form (Visits 1-3, 5-7)
A narrative approach is used for this evaluation form to allow supervisors the opportunity to note how candidates are translating theory to practice, developing strengths and areas for work.
- Early Warning Form
If a candidate is having difficulty or not responding to structured feedback within the first 5 weeks of their field experience, please fill out this form and contact the Office of Field Placement and Supervision. Please provide specific documentation.

Secondary Education
- Midterm Senior Teaching Clinical Evaluation Form
Mandated form to be completed by the Cooperating Teacher and the University Supervisor at the time of the fourth visit; a final grade is not associated with this form.
- Final Senior Teaching Clinical Evaluation Form
Mandated forms to be completed by the Cooperating Teacher and the
University Supervisor at the end of the senior clinical experience. A
final grade for the experience is recorded on this form.
