**Please note: The evaluation forms listed here
are provided for review. The required evaluations are provided to you,
in a scannable format, in the information packet you receive from the
candidate working in your classroom.
Interns
- Intern Cooperating Teacher Handbook
The Handbook provides essential information for Cooperating Teachers working with all undergraduate candidates in the following programs: Elementary/Special Education, Speech and Language Pathology, and Secondary Education. The requirements and expectations for the four field internships are detailed in the handbook.
- Early Warning Form
If a candidate is having difficulty or not responding to structured feedback within the first 35 hours of their field experience, please fill out this form and contact the Office of Field Placement and Supervision. Please provide specific documentation.
Elementary & Special Education/Speech & Language Pathology
Secondary Education
- Secondary Program Preclinical Evaluation Form
Mandated tutoring evaluation form must be completed by the University Supervisor for both semesters of tutoring (Preclinical Field 1 and 2).
- Final Junior Teaching Clinical Evaluation Form
Mandated form to be completed by the Cooperating Teacher and University Supervisor at the completion of the junior 60 hour internships (3 and 4).

Student Teachers
- Cooperating Teacher Handbook
The Handbook provides essential information for all Cooperating Teachers working with candidates completing their culminating clinical experience from any of the following programs: Elementary/Special Education, Speech and Language Pathology; Secondary Education, and EPICS. The requirements and expectations for the clinical experience are detailed in the booklet.
- Honorarium & W9
These forms must be filled out in their entirety and submitted to the Office of Field Placement and Supervision to receive the Honorarium and Professional Development hours rewarded by SHU for working with our senior clinical candidates.

- Midterm Senior Teaching Clinical Evaluation Form
Mandated form to be completed by the Cooperating Teacher and University Supervisor at the time of the fourth visit; a final grade is not associated with this form.
- Final Senior Teaching Clinical Evaluation Form
Mandated forms to be completed by the Cooperating Teacher and University Supervisor at the end of the senior clinical experience. A final grade for the experience is recorded on this form.
